Office Hours
Appointments are typically 45-60 minutes in length, with frequency based on individual need. My office hours are as follows:
- Tuesdays 7 - 2pm
- Wednesdays 8:15 - 8pm
- Fridays 7 - 6pm
- Most Saturdays 7 - 4pm
Professional Fees
- Initial Intake Session(s) 1-2 sessions as necessary (60 minutes) - $140.00
- Family Sessions (45-60 minutes) - $120.00
- Individual Sessions (45 minutes) - $100.00
- Individual Sessions (60 minutes) - $120.00
- Missed appointments with no notice of cancelation - I don’t charge for the first missed appointment, the second missed appointment is $25 and $80 is charged thereafter. Repeated missed appointments will result in need to schedule appointments same week and discussion about ability for ongoing treatment with this provider.
- Less than 24 hrs notice for cancelation - I don’t charge for the first missed appointment, the second missed appointment charge is $25 and $80 is charged for each missed appointment thereafter. I consider broken appointments individually and understand that emergencies occur.
- Returned check fee/NSF Credit Card Fee - $25.00
- Phone call/consultation lasting over 10 minutes will be charged at $25 for every 1-15 minute segment after the initial 10 minute period. (If an extended discussion is needed I encourage you to make an appointment. There are occasional circumstances when it makes sense to have a phone session, however, please be aware that insurance companies do not cover this option).
- Court Appearances - $250 per 1 hour (travel time, wait time and court time will be billed. If court is canceled a minimum of 2 hours will be billed. 2 hours of payment is required in advance.
- Request for letter writing or completion of paperwork such as FMLA or disability assessments that are completed outside of session start at $25.00. This depends on the nature of the request and time required to complete the request. Copies will be charged at .10 per page
Insurance
I accept most major insurance plans. Services may be covered in full on in part by your health insurance or employee benefit plan. Co-pays, co-insurance, and deductibles are due at the time of the session. You may want to contact your insurance company to verify benefits and coverage prior to your initial session, using the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
